Madison Central Marching Band Recommended Contribution for 2020

  • I hereby agree to payments totaling $300 for the 2020-2021 school year according to the following detailed contribution schedule.
  • Please sign and return this Recommended Contribution Agreement along with the first contribution of $100.00. This completed agreement signifies contributions are due by the dates indicated above. If you believe you are unable to fulfill the terms of this agreement, please submit the Hardship Application ASAP.

    There is a sibling discount available of 50% to families with a second student.

    I have read and understand that I am responsible for the contributions listed above. I agree to pay the contributions listed above with cash, check ( payable to Madison Central Band Boosters ), on or before the due dates. The following information should be completed by the party responsible for the recommended contributions for the band expenses.
